脊椎關節病變（英語：spondyloarthropathy，縮寫為 SpA）泛指任何影響脊椎關節的關節病變[1]，它是一大類疾病的集合總稱，而非單一個特定疾病。脊椎病變則是脊椎骨本身的問題，兩者並不相同。

發炎造成的脊椎關節病變稱為中軸脊椎關節炎（英语：axial spondyloarthritis）[1]。最廣義的定義，脊椎關節病變泛指影響到脊椎的任何關節疾病（包括類風濕性關節炎和骨關節炎），但這個名辭通常是指具有某些共同特徵的特定類型疾病，這類疾病又被稱為血清陰性脊椎關節病變。患者有較高比率具有 HLA-B27（英语：HLA-B27） 這個基因，且類風濕因子（RF）與抗核抗體（ANA）皆是陰性。著骨點病變也可能伴隨這類疾病出現。

血清陰性脊椎關節病變

血清陰性脊椎關節病變（也稱為血清陰性脊椎關節炎）是一類侵犯中軸骨疾病的總稱[2]且這些疾病的血清狀態（英语：serostatus）呈現陰性。

類風濕因子是一種血清學檢查，檢查的結果稱為血清狀態（英语：serostatus），而血清陰性是指這些疾病的類風濕因子檢查呈性陰性[3]，代表這類疾病的病生理機轉與類風濕關節炎不同。

分類

國際脊椎關節炎評估學會（Assessment of Spondylarthritis International Society，ASAS）提出的中軸脊椎關節炎（英语：axial spondyloarthritis）分類標準（簡稱 ASAS 標準）適用於「腰痛持續時間大於或等於3個月」且「發病年齡小於 45 歲」的患者[11]。還可再細分為兩大類[12][13]：

  1. 影像學檢查有薦腸關節炎，加上一個 SpA 特徵，或
  2. HLA-B27 陽性，加上兩個其它的 SpA 特徵

影像學檢查有薦腸關節炎[11]：

  • MRI 上顯示活動性（急性）發炎，強烈代表 SpA 相關的薦腸關節炎和／或
  • 放射線檢查確診為薦腸關節炎
