Cèl·lula de Langerhans

Les cèl·lules de Langerhans, descrites per primera vegada pel patòleg alemany Paul Langerhans l'any 1868,[1] són cèl·lules dendrítiques,[2] abundants a la zona suprabasal de l'epidermis,[3] presents en la còrnia perifèrica i en la conjuntiva oculars[4] i encarregades de la presentació d'antígens;[5] en les quals hi ha uns grans grànuls anomenats grànuls de Birbeck[6] formats per l'activitat inductora del receptor transmembrana langerina.[7] Fins a mitjans de la dècada de 1960 no va ser identificat el seu paper en la resposta immunitària innata[8] contra la radiació ultraviolada,[9] microorganismes patògens[10] o substàncies químiques nocives.[11] Anys més tard, fou descoberta la particular funció defensiva exercida per aquestes cèl·lules davant determinades infeccions víriques[12] i bacterianes.[13] S'originen en el moll d'os, el factor de creixement transformant beta-1[14] és el principal regulador de la seva proliferació[15] i en estat estacionari o inactiu tenen la capacitat d'autoregenerar-se amb independència de l'hematopoesi normal.[16] Les cèl·lules de Langerhans primordials adultes deriven principalment dels monòcits creats pel fetge fetal i, en menor mesura, dels macròfags primitius procedents del sac vitel·lí extraembrionari.[17]

Són cèl·lules arrodonides, amb un nucli dens excèntric de forma dentada, abundant citoplasma PAS-positiu lleugerament eosinòfil i prolongacions llargues i primes. Mostren positivitat immunohistoquímica al cúmul de diferenciació CD1a[18] i negativitat a la macrosialina,[19] una característica que permet distinguir-les dels macròfags.[20] L'exposició crònica a la radiació solar pot alterar les seves funcions i afavorir la carcinogènesi epidèrmica.[21] Normalment les cèl·lules de Langerhans es troben a la pell[22] i als ganglis limfàtics,[23] però en cas d'histiocitosi se les pot trobar també en altres òrgans,[24] con ara els pulmons,[25] el cor,[26] els ossos,[27] el tracte gastrointestinal,[28] els genitals femenins,[29] l'ull,[30] el fetge,[31] la glàndula tiroide,[32] el tim[33] o el cervell.[34] Estan involucrades en la patogènesi del càncer bucal escatós,[35] del timoma ectòpic amb estroma limfoide,[36] del càncer de pell no melanocític,[37] del carcinoma laríngic de cèl·lules escatoses,[38] del limfoma cutani de cèl·lules T (micosi fungoide),[39] d'alguns tipus de colesteatoma,[40] de la psoriasi,[41] de la granulomatosi pulmonar homònima,[42] del granuloma eosinofílic solitari ulnar,[43] del granuloma eosinofílic orbitari,[44] de la disgènesia reticular (una malaltia congènita i hereditària autosòmica recessiva que afecta la immunitat i que és una de les causes d'immunodeficiència combinada greu),[45] de certs casos de malaltia periodontal[46] i d'hematopoesi extramedul·lar cutània neonatal,[47] de la dermatitis crònica,[48] de la dermatitis de contacte al·lèrgica,[49] del queratoacantoma,[50] de l'estomatitis aftosa recurrent,[51] de determinats fibromes odontogènics[52] del liquen esclerós vulvar,[53] del liquen pla oral[54] i cutani,[55] de certs histiocitomes fibrosos malignes secundaris,[56] de la leucèmia limfoblàstica aguda en alguns individus[57] i de les malalties de Hashimoto-Pritzker,[58] de Hand-Schüller-Christian[59] o de Letterer–Siwe.[60] Rarament, un quadre d'asma incontrolada és la primera manifestació de la histiocitosi de cèl·lules de Langerhans.[61]

En un individu sa representen entre un dos i un quatre per cent de la població cel·lular epidèrmica[62] i la seva densitat, depenent de la zona corporal on es trobin ubicades, oscil·la entre les dues-centes i mil cel·lules/mm2.[63] Amb l'edat, el seu nombre i funcionalitat immunitària disminueixen,[64] fent la pell més procliu a patir infeccions i/o al desenvolupament de lesions neoproliferatives de gravetat diversa.[65] Algunes d'elles, molt singulars, són la variant no calcificada i rica en cèl·lules de Langerhans del tumor odontogènic epitelial calcificant[66] i la neoplàsia nodal de cèl·lules de Langerhans.[67] El sarcoma homònim és una neoplàsia maligna agressiva i poc habitual,[68] que de vegades es desenvolupa a partir d'una histiocitosi prèvia d'aquest determinat tipus cel·lular.[69] Pot ser localitzat[70] o multifocal.[71] Excepcionalment, el tumor infiltra l'interior de l'ull[72] o apareix a un gangli limfàtic axil·lar,[73] a la bufeta urinària[74] o a l'arrel de la llengua.[75] Les lesions cutànies produïdes per aquest tipus de sarcoma responen bé i de manera duradora al tractament radioteràpic.[76] El carcinoma broncogènic associat a la histiocitosi X pulmonar és una malaltia inusual de lenta progressió[77] que afecta predominantment a adults fumadors.[78]
